Cache disabled + external image = no 304 ever

If you disable the cache, you will never get a 304 on remote image since we (well I) use the temp file's mtime. Is this a problem ? I think...

Proposition: Never cache external images that do not send proper cache headers

This would add a load on the server, but would respect what the server asked for.

This could prevent memory exhaustion attacks. See http://php.net/manual/en/function.imagecreatetruecolor.php

Disabling upscaling yeilds image not in good ratio

Let's say I /image/2/2048/1500/image.. but I have a 1920x1080 image. Disable upscaling bails out, but I think we should still respect the 2048:1500 ratio without upscaling... what do everybody thinks...
