Do you mean that `rev` is the base revision on top extensions are enabled and the extension flags are specific to the revision?

This would also help the rust bindings (after #526). I think `uint32_t` is fine considering EIP-1985.

Rebased. I think this is still useful for unit tests, based on `ethereum/tests`. I think I needed/wanted it for soltests at the time.

Shouldn't include `account_exists`?
