I was able to eventually get it working by manually fixing the Input Log file, as the conversion only converted controller 2 to a 6 button input, but not controller...

I think I noticed less issues in SW mode. But in HW mode I still do see this issue. Still, it appears less prevalent than in the OP's original image....
VGMTrans is the reference library I use, and already comes with a few game specific hacks.
You can use VGMTrans to merge VH and VB sounds, I think. But it's not that straight forward.
