@glwagner I ran the tests and they got stuck in the same place where [this test](https://buildkite.com/clima/oceananigans/builds/8208#01821729-56d9-42df-8776-730055a3332e) got stuck. So it appears that there's something to be fixed here...

It turns out this also affects `JLD2OutputWriter`. I believe this is due to us "double-counting" indices in `construct_output()`.
