Yes this was changed in https://github.com/redolution/gekkoboot/ which PicoBoot uses as its default payload. I believe right now you can boot into stock menu by holding D-Pad Left or Reset button.

It's currently not possible but definitely a great idea for a feature.
What kind of HDMI adapter do you have?
This is a hardware issue with the HDMI adapter. To mitigate the problem you should power Pico from 5V but that would be impossible with a flex cable.
