I'd say that the underscores is a minor _style_ issue, whereas the whole file is flawed. It is written in c++98 and it should be rewritten entirely.
Yes, this issue had already been adressed by this commit.
From my experience, bold reminders are ignored by most readers. That is why I keep this current issue open even if in later commits the bug was fixed.

Is the behavior same with a simple #pragma omp parallel for ? (i mean without collapse/scheduling)
