Triggers are server-side unless they are of the "script" type. If there's an issue otherwise (like doors, etc not being activated), then it's a bug.

No. 4 is an engine thing, models can only have one rather simplistic bounding box, so it is either _on_ or _off_. The rest sounds like a bug.
