I've already started the second option. It's not finished, since I faced some problems in the diesel-adapter. After releasing version 0.3.0 it's more possible to be able to finalize it.

I think having a CHANGELOG.md is better, since you can version it and change it, for example, you decide to add some ref links for "Added custom types for fields"....
I would say yes. Because normally we need to run it while deployment, and in case we have several instances of the server, it will be executed several times.
