Polylines slice through 3D Tiles rather than draping over them

When clamping a polyline to 3D Tiles it slices through the mesh rather than draping over the topmost surface. This is a limitation of the way ground polylines are rendered....

Adds a visitor that retrieves the index supplied by `IndexAccessorType`. There's some overlap with `IndicesForFaceFromAccessor ` but this allows for simpler direct access of indices.

Upgrade should remove the leading slash in tile url

If a tile url has a leading slash it should be treated as a warning, if not an error. See https://github.com/AnalyticalGraphicsInc/cesium/pull/6125
