I think you'd have to use qemu or pcem with a copy of dos then.
On windows 10 with civ 1 1.2 they do. 
Win16 programs with theming enabled can't be a fully supported configuration. Ntvdm didn't support it at all, it's just not reliable.
 I don't see this here. It's likely a Windows or video adapter scaling issue.

Printing a ^G to what? A console window? What are you running in winevdm that then hangs? Way more information is needed.
Setting the microsoft pdf printer and creating a pdf works fine here.

They are built in mdi windows but custom one and they explicitly use the default system menu icon. Even if they were real mdiclient windows they'd still use that icon...
