Not all code blocks are annotated with {:lang="js"}

There are a lot of short code blocks in the book that haven't been annotated with {:lang="js"}. Would you like a pull request that encloses all code blocks in {:lang="js"}~~....~~...

I've added {:lang="js"} and {:lang="ruby"} annotations around every code block in the book.

While most of the book uses {:lang="js"}, some sections are annotated with {:lang="javascript"}. I've raised a pull request https://github.com/raganwald/javascript-allonge/pull/93 to fix this.

This is simply for the sake of consistency, as the rest of the book uses {:lang="js"}.
