Fix a bug when QS remove is turn on and on non-cached pages this not need to happen. See trello: remove query string still takes place in no-cache page

Talked to support team and added the new exlusions from PR: https://github.com/litespeedtech/lscache_wp/pull/269 Also I did some cleanup and regrouping of current rows
