This looks a bit like a bug in the original which happens to work by luck. It seems like the player is moved after unlocking the door in the original....

I guess the wrong door index is already fixed but the key consumption should still be adjusted (on both maps to be consistent).

No. Unless you duplicate items like wishing coins though savegame manipulation. I think the mentioned issue is not related to the things you mentioned. This is no "do nothing" reward....
