Sorely needed now that I made VCL the default text role. A whole lot of stuff changed from title references to just italics, and I'm not sure it makes sense.
This is definitely not trivial for the PDF, as rst2pdf isn't too good at this. More experimentation is needed. See also --real-footnotes and --inline-footnotes and .. target-notes::
