Seems like you have duplicate entries in your mod. What's most likely happening is that you are installing a file which is both in a WAD and in a WAD...

Try running Fantome as admin as mentioned [here](https://github.com/LoL-Fantome/Fantome/wiki/Common-Issues)
The mod file (.zip) he provides in the description is not a valid mod that you can install with Fantome.
Delete any non .wad.client files you have in your FINAL Game directory
