Unexpected problem when unmounting

I've got this error reported from an app in production and it seems that it tried to remove view more than once. Maybe a simple try/catch on [L138](https://github.com/zserge/anvil/blob/master/anvil/src/main/java/trikita/anvil/Anvil.java#L138) would solve...

What do you think of having on the Anvil DSL some shortcuts like verticalLayout and horizontalLayout for linearLayouts with respective orientation? Something for the various input types would be nice...

[Suggestion] Name variables according to the fields

First of all, great project! Currently, the variables are named `v1`, `v2`, `v{n}`... For the sake of debug, It'd be great if they're named after the fields. The variable "index"...
