Can't you just use the Overseer Webhook notification instead of the predefined Discord notification to create a custom discord notification that meets your specific requirements?

The 404 error seems really suspicious. If I hit booklore with an invalid url, I get a 403 forbidden response not a 404. It seems almost like you are not...
Did you reauthenticate the plex server by hitting refresh the servers button on the Overseerr settings -> Plex page?
