I can reproduce this issue as described, i.e., repeated uploads of the same image can result in restrictions not applied consistently.
I do not think this is an RC4 requirement. In my mind, when I logged this, it was a longer-term consideration. Can we remove the RC4 label @MissAllSunday ?

It's necessary to properly identify the alert. A "topic reply" that only ids the topic isn't specific enough to uniquely the message that was approved. Suzie might have 2 unapproved...
