This reapplies https://github.com/ros2/rmw_connextdds/pull/132, which was reverted by https://github.com/ros2/rmw_connextdds/pull/146, with the required bug fixes.

A different draft for this PR was submitted in https://github.com/ros2/rmw_connextdds/pull/147. We're actively working on this fix now, so we're opening a new PR from a more up-to-date branch which also follows our naming scheme.
