Static QR codes. For Dynamic QR codes you would need to encode a redirect URL, which you can then change as needed, but that isn't part of this javascript program...
Try changing dot scale from 0.35 to 1. I have no problems scanning the QR Codes.

Same problem here with Safari, Firefox or Chrome on iPhone.
