Missing LICENSE file or license headers or license information in README

Dear gzweb devs,

I just noticed that there is no repository wide license information. Most C++ files have the Apache 2.0 license header, however none of the Javascript files contain one.

Would it be possible to either add a LICENSE file, add license information to the README or add license headers for all files?

@chapulina or @scpeters this seems like a quick thing to fix directly from GitHub's UI to create a new file named LICENSE and then it will prompt to choose a license from template.

I don't know what the etiquette is on submitting a PR with a License to a project without a License.

I believe it should be Apache 2. We're also missing copyright headers on most files. What do you think, @iche033 ?
