I am having the same issue and as you say the problem is the hardware decode. See https://github.com/Revan654/staxrip/issues/90 and https://forums.developer.nvidia.com/t/nvdec-fails-on-file-requires-29-threads-surfaces-thus-fails-on-3-threads/240827/4 I believe you need to specify nvenc as the hardware...
Didn't change anything. export QT_FFMPEG_DECODING_HW_DEVICE_TYPES=vaapi export QT_DISABLE_HW_TEXTURES_CONVERSION=1 in the plasma-workspace/env directory had no effect.
