Something in **query_vars** function is wrong that's because i've used this kind of _hacks_ at beginning of **filter_request** function
Since Wordpress 3.5 this is relatively more easy to implement. Like posts, translate all attachment fields, **title**, **caption**, **alternative text** and **description**. And of course the **slug**.
Rebuild this qtranslate function to print correct permalinks. _Sorry, this entry is only available in %LANG:, : and %_
