Have you considered using [well-known URIs](https://tools.ietf.org/html/rfc5785) (with associated [registry](https://www.iana.org/assignments/well-known-uris/well-known-uris.xhtml)) and include a registration for a well known URI `/.well-known/health` ?

Off the top of my head [ACME Challenges](http://www.iana.org/go/draft-ietf-acme-acme) stands out as the one big success story. The well-known registry itself has numerous entries, so presumably it's being used, but like...
