[BUG] Sub-agents claim successful file creation but files don't persist to filesystem

I encountered this issue on Ubuntu ([here](https://github.com/anthropics/claude-code/issues/7476)). I tried 3 different terminals, both within and outside of an IDE. It made no difference.
