Extensions being deprecated in favour of plugins in the next major release of Apollo Server.

Hey, thanks for creating this extension, very useful. When running this extension in Apollo server 2.15.0, you get the following warning `[deprecated] A "NewRelicExtension" was defined within the "extensions" configuration...
