I have the same error before, I fixed it by publish my notion page, publish button is inside the share button of notion page.

For anyone inerested, TOC can be fixed by [HandyOutliner](https://handyoutlinerfo.sourceforge.net/). Run `OutlineTool.exe -s:"source.pdf" -t:"target.pdf"` to copy TOC from source.pdf to target.pdf.

Only the first breakpoint works

Found the reason, directory seperator doesn't seem to match. Inside my breakpointsPerPath: ![image](https://user-images.githubusercontent.com/4527817/34926938-c8528b2e-f9ed-11e7-9662-b8e76b3b80a9.png) And my env is win7 + lua5.3
