Those aren't the steps to reproduce. Use the Settings (formerly Preferences) button in the Safari Extensions Settings pane.

Fixed in iOS 18.4

This initially appeared to be fixed, but it's actually not fixed. I've filed a new Feedback FB17089367. The extension is enabled immediately after updating, but after it delay it automatically...
