do headers need to be in first row?

I think we are assuming that the data files have headers in the first row - do we want to support cases where that is not the case?

Handling for postgres schemas?

Would it be possible to support Postgres databases that have tables in schemas? I'm running into problems I think because table names have to be prefixed by the schema name...
