Allow boot from volume

For bigger deployments it would be nice to have an option to use "boot from volume" nova option when creating instances instead of passing an image.

affinity filters have to be explicitly enabled in OSP9

In OSP9 packstack deployments nova scheduler doesn't include by default following filters: "ServerGroupAntiAffinityFilter,ServerGroupAffinityFilter" and these have to be added explicitly. It would worth adding a note into prerequisities about this.
