Backend response headers set issue

We had a report that this was causing duplication when using `beresp.headers.set()`. We should ensure we either properly guard or fix this case.

We could support a similar static method for backend construction, see https://github.com/fastly/ExecuteD/pull/4378.

This could be a nice feature to more easily get the SDK version directly from the JS error stack.

We still need to support these properly for the cache hostcalls on cache lookup and insert.

We should maintain a benchmark suite like the integration test suite that can run on CI and that can provide relative benchmarks with changes over time.
