I just did a test with mesa-nouveau and no issue with 240p suite when Full FB Emulation is enabled.
Try disabling VSync in Settings > Video. And make sure no external utility is forcing VSync on Flycast.
In your case, the middle screen (Slave 1) isn't even visible. Did you stop it, or is it behind the master screen? If the issue is intermittent, it's probably not...
