File-level doc blocks are missing in the output

None of the text or tags in a file-level doc block make it into the output (using default settings). Minimal test file:

The man page section for `get` currently reads: ``` get [-E] [-a] [-c] [-e] [-P N] [-O base] rfile [-o lfile] ... (...) -P N download N files in parallel...

Links with references cannot be inserted in Markdown mode
