Check out [gd2md-html](https://github.com/evbacher/gd2md-html/wiki), a full-blown Docs add-on inspired by Renato's original here. List processing (and many other things) are much improved. Thanks to Renato for pioneering this conversion work!

Yeah, when I originally wrote this add-on, we were generating a TOC server-side (via some JavaScript), triggered by the [TOC] directive. I don't have access to that server or code...
