Could not recreate this issue on Approvals v1.6.3 Checkout does not proceed, and a notice is shown if the user does not have approval for the first level. 
Was able to recreate this issue on PMPro 3.4.4 and PMPro Approvals 1.6.3. Approval-specific variables e.g. _!!member_name!!, !!membership_level_name!!,_ etc. are not replaced in Approval test emails.
