It happened several times to me that the same item was offered multiple times in the same item roll. Not sure if this is intentional, but it is a poor...

 When the opposing pokémon hold too many items, it is no longer possible to read the sp. atk. IV of the pokémon you just caught. Also the Yes/No option...
