REQUEST: Add posibility to define http_header_user

Make it posible to apply the following fastcgi_param REMOTE_USER $http_X_User; into apache configuration. So its posible to use proxy authentication

When using Universal Login with design and logo set, then forward auth0 is still using default login logo for auth0
