FWIW PR #134 doesn't add the Adobe-specific page-map element, but it does add the [Epub 3 page-list](http://www.idpf.org/epub/30/spec/epub30-contentdocs.html#sec-xhtml-nav-def-types-pagelist), which accomplishes the same thing and is supported by Adobe Digital editions.
