As soon as the SWIFT command is issued, boom, segfault.

not sure if darren is keeping up with this code, try my fork @ git://github.com/jkister/app_swift.git

fyi, i just got the fork working, seems to behave on Asterisk 11 and Asterisk 1.8. it's probably broken on asterisk 1.4/1.6.
