Does this respect the [AND] and [OR] Operators?
This is not an issue of Cassiopeia. You can style your layouts as you want, with own classes or in your user.css or via an override.
Meanwhile the Input - button issue looks like that: 
I would like to have a landmark here and get rid of this ugly "missing landmark" error. But am not sure if
I cannot replicate the issue. In J5 and J6 all mails are sent.
There coud be an override of the lost password page. But perhaps it is only a typo in the mail-address?
Which template are you using?
The super user gets a message that he cannot require a new password.
