Add support in ValueSet generators for generating R4 ValueSets

Need to analyze existing operations to determine where this is needed as well as determine if the operations are still relevant.

FlatMultiValueSetGenerator uses name for file name, should use id

Still need to ensure that the filename is using the id instead of the name.
