Add support in ValueSet generators for generating R4 ValueSets

Is your feature request related to a problem? Please describe. Valueset generation currently creates STU3 valuesets. For example VSACValueSetGenerator does not have a -version option to specify STU3 or R4 and currently creates STU3 valuesets. There are -vc and -mvr options, but neither are for which FHIR version to create.

Describe the solution you'd like Add a -v option to allow specifying which FHIR release to create valuesets for.

Describe alternatives you've considered It can be done by hand, but it would speed development up if we had this built in.
