Pull request #176 addresses this and looks close to completion. They just need to figure out how it should behave and be tested on older versions of node.

Sharp strips all metadata by default, including the Exif orientation tag. There is a [`rotate()`](https://sharp.pixelplumbing.com/api-operation#rotate) option than can be used to rotate the output image to match the Exif orientation.
