Installing SSMS 17.9 alongside 18.2 causes the extension to work! Going to do that until it's fixed; I missed this tool so much.

SSMS 19 - Compatibility

I've used this tool with SSMS 18, 19, and now 20 just by installing [Visual Studio 2015 Shell (Isolated)](https://visualstudio.microsoft.com/vs/older-downloads/isolated-shell/). No need to modify assembly bindings.
