I have been using dev build https://github.com/retrooper/packetevents/commit/5247ccd12941421a0691fe94afc91821073de99b for a week and have not seen that error again 👍
This crash seems to be caused by a deadlock between a Netty EventLoop thread and the Server thread caused by some ProtocolLib listeners from ItemsAdder that cause off-main chunk lookups
I've ported some patches from Paper that seem to fix this problem
Closing because it causes conflicts with Lithium, I'll pull the latest changes and fix the conflicts and create a new PR soon.
