I'm having this bug, too. And i'm on Mac OS X. I confirm that it occurred, like AryaStark99 said, along the UI change: while they put in some really useful...
Just a quick note: when a URL gets the "file access error" it will *always* get it, even if you retry or delete and add it again
Maybe i've been unlucky, or i stopped retrying too early :), but i picked a couple of dozens of failed URLs and even after 20-30 retries i keep getting this...
Don't know why i keep reporting, since looks like nobody cares, but anyways... Looks like a size-related issue: i only get this problem with files between (approximately) 1 and 2...

YES! I left untouched the start order, but changing the other two parameters fixed the issue for me as well. Within the next couple of days i'll try reverting them...
