I remember it happening in the Dark World theme from A Link to the Past, and possibly the Athletic theme from Yoshi's Island, but I'm less sure about that one.
Update: Just found another example of this: the Battle Arena theme from Pokemon Emerald (song 458) has the same issue with its squares (channels 7 and 8).
