@aaronntw make sure to add it to `configuration.yaml` only, not the HA addon config.

Could you try to re-configure the device and provide the debug log of that, afterwards restart z2m. See [this](https://www.zigbee2mqtt.io/guide/configuration/logging.html) on how to enable debug logging.
