It looks like your 'Build and Deploy' workflow didn't actually run. I'd try disconnecting GitHub from the deployment center and then reconnecting it.

@jn1102 Check to see if you have a Per-User MFA service setting to remember devices for 90 days. If not there, maybe a conditional access policy with an inactivity limit.
