Resizing the side panel sufficiently widely seems to fix this: 
In Chrome 127.0.6533.122 and 128.0.6613.85, I don't see the inconsistent icon placement shown above, but I do see the file name column being unusually narrow by default. 
I noticed as well that, in Firefox, selecting different filenames causes the columns to change in position. 
Per discussion on the frontends meeting, let's resolve this by documenting expectations about where and when to scroll the notebook after a cell operation, for developers' and users' benefit.
FYI: #16646 has been merged in, so now is a fine time for a follow-up PR.
If I add `flex-grow: 1` to the style of both the `jp-DirListing-itemName` and `jp-DirListing-headerItem` classes, in Firefox, I see a layout that provides ample space for both "Name" and "Modified",...
Setting the "modified" column's minimum width to 100px will accommodate the "Modified" column in English, and also avoids having ellipses in any of the compact relative dates.
@stefansimik Thanks for opening this issue! This may be a duplicate of #16371 — no HTML tags are interpreted at all in the TOC.
@krassowski points out that LaTeX expressions, like `$R_2$` or `$R^2$`, are also not supported in the TOC: #15047
